The Gathering Place (Brattleboro Area Adult Day... is Hiring an Intake Coordinator (LPN) Near Brattleboro, VT

The Gathering Place (Brattleboro Area Adult Day Services), is an Adult Day program serving Windham County Vermont. We serve adults with a wide range of physical and cognitive abilities. Our participants enjoy music, arts, games, exercise and companionship - just to name a few!

We are seeking an individual to join our team in the role of Intake Coordinator (LPN).

The Intake Coordinator is responsible for all facets of the intake and enrollment process including serving as a primary point of contact for referrals and performing assessments (physical, social, etc.)

This role also serves as a clinical resource to our program staff and provides routine and preventative nursing care (health monitoring, medication administration, vitals, health education, etc) and assists participants with ADL's and activities.

Some of the key duties you will be responsible for:

  • Responding to referrals and requests for information and conducting tours.
  • Ensuring all enrollment paperwork is complete and accurate and following up on incomplete/missing information with internal and external sources.
  • Providing clinical support, insight and knowledge to program staff and providing routine and preventative nursing care to participants.
  • Working collaboratively with the Program Director/RN on person-centered plans of service and participant documentation.
  • Assisting activities staff with maintaining appropriate staff-to-participant ratios and performing/assisting participants with personal care and ADLs.
  • Communicating effectively with physicians, families, and caregivers.
  • Reporting monthly on enrollment and inquiry data.
  • Participating in discharge planning when appropriate.

This is a full-time hourly position - 32-37.5 hours per week (flexible for right candidate). After orientation, required schedule will be 7:30AM start, Monday-Friday. We are closed on all major holidays - no evenings or weekends.

Starting rate of pay is $22-27 per hour depending on experience.

Requirements

  • LPN with current Vermont State license
  • Sufficient experience to perform the essential functions of the position with or without accommodation.
  • At least one year nursing experience working with elders and adults with disabilities and their families. Experience in human services, mental health, memory care/dementia ideal.
  • Nursing assessment skills.
  • Computer skills required. Familiar with Outlook, Word, Excel and ability to learn new programs. Comfort with web-based computer programs.
  • Effective written and oral communication skills.
